About PLANS

We know that information and courses designed to support people with long term conditions are not always helpful because they often do not take account of people's day to day life and circumstances.

How people manage long term conditions (such as diabetes, chronic kidney disease or chronic heart disease) is influenced by many things, such as feeling alone, being unemployed or caring for a relative.

The support available to people outside the health service (in particular local community organisations and groups and activities such as walking for health groups, art groups or lunch clubs) is being recognised as important in the management of long term conditions as it allows people to think about how to manage their health within the setting of their everyday life.

PLANS has been developed to help individuals to find appropriate local activities to support them to make lifestyle changes, such as diet or exercise, and make the changes part of their everyday lives as well as to find fun and rewarding local activities.
